Credit Application
If you have not purchased from Billboard Bottles on terms in the past, please
complete and return a Credit Application. We have made the application
available to view and download as an Adobe Acrobat document. Please
follow the instructions below to speed through the credit application
process.

Print, Complete, Sign and Fax...
1)
Viewing,
downloading and printing of the Billboard Bottles Credit Application
requires the Adobe Acrobat Reader software. If you do not have
this software on your computer, it is available free from Adobe.
Just click on the link to the right and follow the instructions.

2) Once the Adobe
Acrobat software is loaded on your computer, press the link to the
right. This will open a new browser window and display the
application.

3) Once the Credit
Application document is open, you may print it directly to your local
printer or save it to your local computer's disk drive from where you
may print it at any time. To print the document immediately,
from the menu bar in the Adobe Acrobat window, choose File - Print.
Select your printer and click OK. To save the document to your
local computer disk, again from the Adobe menu bar, choose File - Save
As. Choose the folder where you would like to save the file and
click Save.

Once printed,
complete all required sections of the Credit Application.

Sign the
application and fax to the Billboard Bottles accounting department at
814-669-4528. |
